    Not medical but when I had a UPS account for my businesses I was charged a fee to access my account on line. One of many reasons I dumped UPS.

    That may have changed, depending on how long it's been. It's probably cheaper for the company for people to access online than it used to be.

    I meant to cut out the "government buys" part, because this has as much to do with insurance, including private insurance, as it does government, so I think the premise of the quadrants is kind of beyond dispute.

    Was about a year ago. My main grip was there were four other extra charges for pick up for my businesses. None with USPS. Guess who gets my business now?

    Doesn't insurance usually challenge stupid crap from hospitals? Procedures have a price, then there is the negotiated price that insurance will pay.
