    Why?

    Because of better birth control.

Americans are at least 50 years behind any other first world nation on birthing care, and we have the mortality rates to prove it.
Giving birth is not a "medical emergency". It's as natural as any other female-only function.
There are other factors in play. For instance, look at average age of women giving birth. Look at the prevalence of medical conditions, such as diabetes and obesity. Look at the percentage of the population that has access to affordable health care. The United States stands out in ALL of these variables, and as such, it seems wrong to place the blame on "a lack of midwives."

Because the act of providing abortion was demonized. Even after Roe, midwives were vulnerable to arrest for the unlicensed practice of medicine.

Better midwifery. Better prenatal care. Very few births outside hospitals that present a risk of going sideways.

Our great-grandmothers gave birth in a field and went back to hoeing afterwards.

Birth is not usually medically risky.

This is the joy of abortificants. In the first trimester, no one need know but the female and her Google search engine.

Statistics on abortion are hotly debated. If that did occur, it's likely due to greater access to B.C.

Nope. It's still our medical care. You can weigh for all those variables and you will still see the US doing worse than, say, Panama.
