Here is a mishap that is likely not well known. Multi megaton warhead on missile when missile blew up.
There is a documentary on this subject on PBS called "Command and Control".

It's long, maybe 1 1/2 or 2 hours.

It runs occasionally on PBS and may be available on the interweb.

This was a bad event, but consider the "Broken Arrow" events...losing an H bomb in an aircraft event. All the thousands of warheads out there, it's a miracle one hasn't gone off.

On 26 September 1983, the nuclear early-warning system of the Soviet Union reported the launch of multiple USAF Minuteman intercontinental ballistic missiles from bases in the United States. These missile attack warnings were correctly identified as a false alarm by Stanislav Yevgrafovich Petrov, an officer of the Soviet Air Defence Forces. This decision is seen as having prevented a retaliatory nuclear attack based on erroneous data on the United States and its NATO allies, which would have probably resulted in immediate escalation of the cold-war stalemate to a full-scale nuclear war. Investigation of the satellite warning system later confirmed that the system had malfunctioned.

A few have been lost, and never found.

By some reports, the US has lost 8 of them.

No idea how many the Russkies have lost.

They are still out there...

I was kind of a young pup back in 80, but Good story / video nevertheless IMO.
My son is actually stationed on a base with the MM ICBM’s. They get kinda uptight with their security around those missiles. :neutral:

It will happen eventually. Probably in our life time.
